I have a user, whose outlook inbox shows 168 unread emails while his blackberry inbox show only 38 mail. When we delete emails on the blackberry they get deleted on outlook, and when we delete items on outlook they get deleted on blackberry.

Delete on is set to mailbox and handheld
on conflicts mailbox wins
cache mode in outlook is unchecked
empty deleted items on exit is unchecked.

Emails settings are fine.

Please advise on this.........

Maybe he just has less email on his BlackBerry....?

What is the date of the oldest unread message for both?

the oldest unread messages are of 15sept 08 in outlook and 25 oct 08

So...that's a 10 day difference....with less days worth of email on the device. Therefore the difference is the unread count.
